Developing person-centred care for chronic conditions in Wales

Take part in an interview to help the Welsh Government (Senedd Cymru) understand how to improve care and services for people living with one or more chronic conditions

What’s the project about? 

The Senedd's Health and Social Care Committee is conducting an inquiry to understand how they can deliver holistic, person-centred care for people living with chronic conditions. They also want to know how local NHS and social care services can better meet the needs of people with multiple conditions.

The Senedd defines a chronic condition as "a broad range of health conditions which often cannot be cured but can be managed with the right support and treatment." This includes secondary breast cancer, and other conditions which may arise as a side effect of breast cancer treatment. 

They’re conducting interviews with people across Wales who are living with chronic conditions to capture their views on these topics.  

Who can take part? 

You can take part if you live in Wales and live with any chronic condition. This includes secondary breast cancer, and conditions arising from breast cancer treatment. 

Why do you want me to be involved? 

By taking part, you can ensure that the recommendations that are made to the Welsh Government are informed and shaped by people living with chronic conditions. This could mean that any changes made will meet the needs of more people and deliver better care.  

What will you ask me to do? 

You’ll be invited to have an interview with the Senedd’s Citizen Engagement Team. The interview can be either face to face or online, and you can let the team know if there’s anything that will make participation easier.  

During the interview, you’ll have the chance to talk about how holistic, person-centred care can best be delivered for people with chronic conditions. You may also be asked how you think health or social care services can better meet the needs of people with multiple conditions.
